5 Common Iron Deficiency Mistakes That Keep Ferritin Low

If you’ve been taking iron supplements for months, but your fatigue and brain fog haven’t improved, your hair is still thinning, and your ferritin levels remain low, you may be making one of several common mistakes that can prevent iron levels from fully recovering.

In my practice, I see many patients who have been struggling with iron deficiency for years. The good news is that once these obstacles are identified and corrected, many people finally begin to see lasting improvement.

Let’s look at the five most common iron deficiency mistakes and what you can do about them.

Why Low Ferritin Matters

Most of the patients who come to my clinic are dealing with gut health or thyroid issues, and low ferritin is a common finding.

Ferritin represents your body’s stored iron. Even when other iron markers appear normal, low ferritin can contribute to symptoms such as:

  • Fatigue
  • Brain fog
  • Hair thinning or hair loss
  • Poor exercise tolerance
  • Dizziness
  • Weakness
  • Difficulty concentrating

Many people focus solely on taking an iron supplement without addressing the factors that may be preventing their ferritin levels from improving.

Mistake #1: Taking the Wrong Type of Iron

One of the most common issues I see is patients taking an iron supplement that they cannot tolerate.

Many physicians recommend ferrous sulfate, but many people report side effects such as:

  • Nausea
  • Stomach pain
  • Constipation
  • Digestive discomfort

As a result, patients often stop taking it consistently.

Some popular formulations may also contain ingredients such as dyes, lactose, or other additives that may not be ideal for sensitive individuals.

In my practice, I typically recommend iron glycinate because it is generally well absorbed and tends to cause fewer gastrointestinal side effects.

When patients tolerate their supplements better, they are much more likely to take them consistently and experience results.

Mistake #2: Not Taking Enough Iron

Another common mistake is taking too little iron to effectively replenish depleted iron stores.

For example, I recently worked with a patient whose ferritin level was only 11. She was already taking iron glycinate, but her doctor had instructed her to take just one capsule daily, providing approximately 27 mg of iron.

After months of supplementation, her ferritin level still wasn’t improving significantly. She was then told she might need an iron infusion, which she hoped to avoid.

A common therapeutic dosage for iron deficiency is often in the range of 150 to 200 mg daily, depending on individual circumstances and healthcare provider recommendations.

Because iron glycinate is generally more absorbable, I often recommend between three and six 29 mg capsules daily, depending on the severity of the deficiency and the patient’s needs.

Another option is alternate-day dosing. For example, someone taking three capsules daily may take six capsules every other day.

Foods and Supplements That Can Block Iron Absorption

When taking iron supplements, avoid consuming them at the same time as:

  • Coffee
  • Tea
  • Milk
  • Calcium supplements

These can interfere with iron absorption and reduce the effectiveness of supplementation.

Mistake #3: Stopping Iron Too Soon

Many people stop taking iron based on serum iron results alone.

I worked with a patient who had struggled with iron deficiency for three years. After starting iron supplementation, her blood work showed elevated serum iron levels, so she was told to stop taking iron.

However, her ferritin remained low.

This is an important distinction.

Serum Iron vs. Ferritin

Serum iron reflects more recent iron intake and can be influenced by:

  • Recent supplementation
  • Meals consumed in the previous few days

Ferritin, on the other hand, reflects stored iron reserves.

A person may have:

  • High serum iron
  • High iron saturation

while still having:

  • Low ferritin
  • Depleted iron stores

Iron supplementation often needs to continue long enough to normalize:

  • Ferritin
  • Red blood cell count
  • Hemoglobin
  • Other iron-related markers

Only then should adjustments be made based on follow-up testing.

Mistake #4: Not Retesting Often Enough

Proper monitoring is essential during iron replacement.

After starting supplementation, I generally recommend retesting:

  • At one month, initially
  • Every three months during the first year

This allows dosage adjustments based on how the body is responding.

For example, one patient recently returned after one month of treatment. Her ferritin had increased from 11 to 19, but progress was slower than expected because she was taking only half of the recommended dosage.

Her supplementation plan was adjusted, and she will retest again at the three-month mark.

What Is an Optimal Ferritin Level?

Every patient is different, but I often aim for a ferritin level between 50 and 80 ng/mL.

Once ferritin reaches an optimal range, supplementation can often be reduced to avoid excessive iron accumulation.

Important Tests to Monitor

A comprehensive evaluation may include:

  • Ferritin
  • Serum iron
  • Percent saturation
  • TIBC (Total Iron Binding Capacity)
  • CBC (Complete Blood Count)
  • CMP (Comprehensive Metabolic Panel)
  • Lipid panel
  • TSH
  • Free T3
  • Free T4
  • CRP
  • LDH
  • GGT
  • Insulin
  • Hemoglobin A1C
  • Uric acid

At a minimum, follow-up testing should include:

  • Ferritin
  • Serum iron
  • Percent saturation
  • TIBC
  • CBC
  • CMP

Mistake #5: Ignoring the Root Cause

Perhaps the biggest mistake is focusing only on replacing iron without asking why the deficiency developed in the first place.

Low ferritin is a clue. It is not a complete diagnosis.

Common contributors to iron deficiency include:

Poor Dietary Intake

Some people simply are not consuming enough iron-rich foods to meet their body’s needs.

Medication Interference

Acid-blocking medications can interfere with iron absorption and make it difficult to replenish iron stores.

Blood Loss

Potential sources include:

  • Ulcers
  • Crohn’s disease
  • Colitis
  • Excessive menstrual bleeding

Heavy menstrual bleeding should never be dismissed as normal without investigating the underlying cause.

Gut Dysfunction and Malabsorption

In my experience, gut issues are one of the most common contributors to chronic iron deficiency.

These may include:

  • Gluten sensitivity
  • Leaky gut
  • Candida overgrowth
  • Bacterial overgrowth
  • Parasites
  • Chronic gut inflammation
  • Malabsorption

When the digestive system isn’t functioning properly, it becomes much harder to absorb not only iron, but many other essential nutrients as well.

How the FreeDiet® Supports Gut Health and Nutrient Absorption

Because gut dysfunction is so often involved, I frequently use the FreeDiet® to help patients improve digestive health and reduce inflammation.

The FreeDiet® removes many foods commonly associated with inflammation and digestive dysfunction, including:

  • Gluten
  • Grains
  • Sugar
  • Yeast
  • Dairy
  • Eggs
  • Soy
  • Legumes
  • Nightshades
  • Processed foods

By calming gut inflammation and supporting digestive health, many patients experience improvements in nutrient absorption and overall well-being.

Supporting Health with the Functional Five™

I also frequently recommend the Functional Five™, which includes:

  • Multivitamin
  • Fish oil
  • Vitamin D with K
  • Magnesium
  • Probiotic

These foundational supplements can help support overall health, nutrient status, and digestive function while addressing underlying health concerns.

The Bottom Line

If your ferritin remains low despite taking iron supplements, remember these five common mistakes:

  1. Taking the wrong form of iron
  2. Taking too little iron
  3. Stopping supplementation too soon
  4. Failing to retest frequently enough
  5. Ignoring the root cause

Low ferritin is often a symptom of a larger issue. Correcting the deficiency is important, but identifying why it developed in the first place is the key to lasting results.

I began treatment under Dr. Tom and Dawn in early August. I had a shopping list of complaints. Many of them I'd been dealing with for 25 to 30 years. Traditional doctors told me again and again that I was perfectly healthy, implying that much of this was in my head. Dr. Tom ran numerous tests based on his hunches, and I'm glad that he did. Tests revealed many things never uncovered by any of my previous doctors. I began feeling relief from some symptoms within a few days and gradually began feeling relief from most others. I'm about 5 weeks into the program and feeling so much better. I had concerns about sticking it out because it is definitely a vastly different way of doing things and it is quite an investment. But there is a light at the end of the tunnel. This is the first time in my many years of trying to get better that I can actually see that light. I am excited to update in another few months because I feel certain that I'll be able to report that I am fully healed. Thank you Dr. Tom and Dawn (and Angelika). You all are an amazing team and I feel blessed that you all are working so hard to get me well. Thank you!!

I am beyond grateful for Dr. Tom and his staff!! Kayla, Dawn and Alexandra have gone out of their way to help me and I know my situation hasn't been easy. I came to Dr. Tom with a long list of issues and hopeless. I am proud to say that a year later and I am thriving. I went from not working out at all to working out 4 days a week, plus yoga. I traveled around the country to many holistic doctors and he has been the only way to help me see major progress. My only regret is that I didn't meet him sooner. I feel taken care of and so grateful for my experience with them.

Dr. Tom Rofrano's Natural Medicine Clinic has changed my life. I came to him with an enlarged thyroid nodule, pain in my joints and tendons, increasing weight, imbalanced hormones, a leaky gut and so many other issues that I didn't even know were causing my symptoms. After extensive testing, he put me on a tailored protocol as well as the Free Diet (trademarked) to help clear up my symptoms. I have been on this protocol for 5 months and I have never felt better. I have more energy, reduced stress, overall happiness, no pain in my joints, clear skin, better menstrual cycles, no pms symptoms of irritability or mood swings. I lost 20 lbs and my gut is healing. All with supplements, dietary changes, and environmental changes to my daily life. I emphatically endorse this method and hope everyone seeks him out. He got to the root cause of my symptoms and I am on a path to heal myself holistically without traditional medicine. In addition to Dr. Rofrano, everyone in his clinic is extremely helpful and generous with their time. I have never met a more caring team of support from a clinic. Thank you to Kayla and Angelika and all the NMC support team for what they have done for me. In addition, I don't even live nearby and Dr. Rofrano and his team have worked to ship all my tests and supplies to me, Zoom my follow up visits, and help me find what I need in my area. So grateful to them all. -Amanda Bailey

I was diagnosed with ulcerative colitis in 2001 and have had severe abdominal swelling since 2008. People continuously asked me when my baby was due and I was never pregnant. I didn't want to be in public or go to work because I didn't feel well and I was extremely self conscious of my body shape. Since starting the FreeDiet in March 2021, I have lost 12 pounds. I am no longer bloated and all my coltis symptoms have disappeared. After completing extensive bloodwork and testing Dr. Rofrano discovered that I am allergic to Gluten and Candida. I knew I was allergic to Gluten and knew that I struggled with Candida but never realized I could be allergic to Candida as well. After eliminating anything fermented from my diet, I feel pain free in my stomach. Food is enjoyable once again. I am very happy with my improved health and energy thanks to this diet, supplements and to Dr. Rofrano. My colleagues are commenting and how healthy and young I look. I highly recommend Dr. Rofrano and all his services.

Testosterone gain of 50% in 30 days with NO drugs. DR. Tom’s nutrition and supplement guidance is foolproof, however, it’s up to you to do the work. My bloodwork improved amazingly across the full spectrum. If you want to be healthy and actually see the lab results go to NMC!

I was in a very sickly state with very high blood pressure, Epstein Barr Virus, severe digestive issues, IBS, and constant UTI ‘s that were always back to back. My cholesterol was 244 and my inflammation was off the charts / extremely high!!!! I decided to read “free diet” book written by Dr Rofrano. It made total sense, so I decided to proceed with the blood tests. This is when I discovered all my lab work was in a very poor state. He uncovered a lot of information and made me aware of why my health was poor and why I was feeling badly!!! I started to follow the diet and take the suggested supplements. Within 5 to 6 weeks everything turned around for me. My labs were nearly perfect in all the previously scary areas where the numbers were completely out of range. I am feeling much better and I am raving about the free diet / supplements that Dr Rofrano can offer you for your specific issues. he is gifted to know exactly how to help each person deal with their health issues. I am grateful to God I found him and had the initiative to move forward!!!! PTL !!!!

My health had been declining over the last 5 years. I have consulted with no less than 6 naturopaths. Non of which was able to help me regain my health. After more recent lab tests my Feritin level had skyrocketed at which point now I was worried. Every one I spoke to said the only option was to do leeching (where they draw out your blood periodically). That did not sound like an option. Then I found Dr Rofrano. To be honest I was extremely skeptical as I have spend lots of money and time with other naturopaths only to end up right back at the beginning. However I had nothing to loose (other than $$$) I decided to give him one try before I competely gave up. I can say I am 100% happy!!!... After an in-depth consultation, and extensive testing. Many deficiencies were uncovered not to mention, sensitivities, thyroid, hormones, high cholesterol the list goes on and on. We began a supplement regiment to get things back on track. Its now 6 weeks later. My labs have significantly improved, my cholesterol is now in normal range, my feritin level has significantly dropped and I am feeling a lot better. Its still early and my body has more healing to do. I am ecstatic to say we are on the right track. My labs have never looked better. If you are just not feeling well and really need to find a solution would highly recommend you contact the Natural Medicine Clinic and speak to Dr Tom Rofrano.. you will NOT be dissapointed.
